The Appellant has sought the following information:
1. Provide copies of all the medical claims submitted by the appellant for self-treatment and for treatment of his family members w.e.f 01.07.2012 to 29.06.2021, as per the format given in the RTI application.
2. Provide details of reimbursement of medical claims in respect of indoor treatment of cancer for Rs. 21000/- approx dated 27.03.2015, submitted in continuation of his treatment in Fortis Hospital, Jaipur.
3. Provide details of all the outdoor treatment bills of the appellant and of his family members submitted for reimbursement since 01.07.2012.
4. And other related information.

Submissions made by Appellant and Respondent during Hearing:
The appellant agreed to submit the correct IPO and pressed for the information sought.
The CPIO vide written submissions dated 14.11.2022 submitted that the appellant retired from the post of JAO and earlier also had filed RTI applications. He pointed out that despite being informed orally and in writing the appellant failed to submit a correct IPO. He pointed out that due to COVID the letter could reach the appellant on 09.08.2021. However, ignoring the same he preferred to file a first appeal. He also submitted the delivery details of the letter dated 03.07.2021 which was actually despatched on 07.08.2021. He also informed that the name and designation of the CPIO and FAA are available on the notice board as well as on the website. He further informed that the original IPO was returned and the appellant had not submitted any correct IPO thereafter.
Observations:
Based on a perusal of the record, it was noted that the CPIO vide letter dated 03.07.2021 replied to the appellant that the IPO submitted by the appellant was blank and not addressed properly. Therefore, he was informed that the same should be in favour of “AO Cash BSNL O/o CGMT Jaipur”.
The appellant was therefore asked whether he had sent the correct IPO later, to which he submitted that he shall submit the correct IPO in the light of the written submissions dated 14.11.2022 which was handed over to him during the hearing.
The CPIO shall revisit the RTI application accordingly after receipt of the correct IPO and send a suitable point-wise reply to the appellant following the provisions of the RTI Act.
Decision:
The appellant is given a last opportunity to submit the correct IPO via speed post to the CPIO within 7 days from the date of receipt of the order and the CPIO shall send a suitable reply within 10 days thereafter.
The appeal is disposed of accordingly
